How scheduling works

Appointments are requested through the secure online Patient Portal. Here is the three-step process from intake to scheduling.

Schedule

Choose a time for your initial visit using the Patient Portal scheduler

Policies at a glance

  • Insurance: Coverage varies by plan; copay / coinsurance may apply

  • Card on file: Required for scheduling; used for patient responsibility balance 

  • Superbills: Available upon request (plan-dependent) 

  • Cancellations: Required 48-hours notice; late cancel/no-show fees may apply

  • Paperwork: Forms/letter completion may require a fee
