How scheduling works

Submit Intake: Complete the secure online form with demographics, contact details, insurance (if applicable), and your reason for seeking care.

Patient Portal setup: Once reviewed and approved, you will receive an email with instructions to set up and activate your Patient Portal account.

Request an appointment: Within your active Patient Portal account, you can request appointments through the online scheduler.
